Financial Policies
Please bring your insurance cards to each vist. We will submit claims to your insurance company but all co-pays are due at the time of visit. Please contact our billing office with any questions you have regarding your insurance. You are responsible for informing the practice of any insurance, address or telephone changes. We do accept Visa, MasterCard, Discover, cash and checks. We also offer and accept CareCredit.
